Adult Day Health Care, Jefferson County

Provides services and support through a medical and support model to meet the needs of clients.

Medical Model: Provides care for qualified individuals who live at home but require restorative support services and socialization and allows individuals to stay in their homes. Develops individualized therapeutic programs for each individual based on physician recommendations.

Social Model: Provides social and recreational activities for seniors to assist in maintaining independence, maintains their present level of function and provides respite for families and caregivers.

Eligibility: Seniors needing social and medical supports to help keep them in their homes longer

Required Documentation: Call for required documentation

Fees: Medical Model: Medicaid, Veteran's Administration, Private insurance and Self-pay Social Model: Private Pay, some insurances, and public agencies assistance accepted
