Crossroads Adult Home

(Program)

Provides a safe, supportive 59-bed residence for men ages 25 years and older with mental, physical, or developmental disabilities. Offers both long-term and transitional housing. Residents are provided with case management services, payee services, medication management, three meals and two snacks a day, as well as regularly scheduled activities and outings.
 

Eligibility: Men ages 25 years and older who: - are independent with activities of daily living (bathing, dressing, etc.) - have an active income source - are compliant with medications - have active health insurance - are medically cleared for adult home level of care - have a minimum 90 days of sobriety outside of an inpatient facility - are actively engaged in a treatment plan - have no history of repeated violent crimes or arson - exhibit no physical violence against staff
