Adult and Children HCBS Services

(Program)

Adult services include pre-vocational services, supported employment, ongoing supported employment, education supports, psychosocial rehabilitation, family support and training, and habilitation.

Children's services include psychosocial rehabilitation, pre-vocational services, caregiver family support, community habilitation, supported employment, and day habilitation. 

Eligibility: Children and adults who are deemed eligible by a health home care manager and a Medicaid managed care organization
