Processes Medicaid applications for individuals seeking help paying for nursing home or long term home health care services.

Eligibility: Financial eligibility determined based on income and resource levels

Application Process: Applications may be mailed; face-to-face interview not required; a pre-screening interview appointment is a courtesy service but is not required

Required Documentation: - Birth certificates for applicant and spouse - Social security cards for applicant and spouse - Alien registration card/I-94 card/naturalization papers for applicant and spouse (if applicable) - Marriage certificate, separation papers, or divorce papers (if applicable) - Death certificate(s) of spouse(s) (if applicable) - Veteran discharge papers for applicant and/or spouse (if applicable) - Power of attorney papers - Written authorization to represent applicant (if applicable) - Medicare and other health insurance cards and verification of monthly premiums - Verification of all sources of income for applicant and spouse - Verification of all assets of both applicant and spouse
