Consumer Directed Personal Assistance (CDPA)

(Program)

Allows older and disabled individuals take charge of their home care services by hiring, scheduling, selecting needed activities of daily living are to be completed, and supervising the aides they've chosen.

Activities of daily living (ADLs) are defined asbathing or showering, dressing, getting in and out of bed or a chair, walking,using the toilet, and eating

Eligibility: Individuals with disabilities who: * have Medicaid * reside in New York State * willing and able to self-direct their services, or have a designated representative who is willing to do it * need assistance with daily living activities, home health care, or skilled nursing tasks as part of a long-term health condition or disability

Cayuga, Cortland, Franklin, Jefferson, Madison, Oneida, Onondaga, Oswego, Seneca, St. Lawrence, and Tompkins counties
