Provides primary health care to students at Beaver River School District, Carthage Middle and High Schools, as well as LaFargeville School District.

Carthage Central High School Based Health Clinic

Carthage Middle School Based Health Clinic

Hours: Monday - Friday, 7 AM - 3 PM


LaFargeville School Based Health Clinic
20414 Sunrise Avenue, La Fargeville, NY 13656
Hours: Monday - Friday, 7:30 AM - 3:30 PM


Eligibility: School-aged children attending Beaver River School District, Carthage High and Middle Schools and LaFargeville School District

Application Process: Enrollment forms available on agency website, school nurse's office, or at the SBHC located in the school

Required Documentation: Immunization and medical records

Fees: No cost for visits at SBHC; there will be a charge for prescriptions, lab work, X-rays and specialty appointments
