Neighborhood Advisor Program
(Program)
Provides intensive outreach and referrals to services and support networking to lessen feelings of isolation and to enable seniors 60 years of age and older to experience their highest degree of independence. Services provided by visiting seniors in their homes, helping them fill out forms such as HEAP Medicare, SNAP, SSI, etc., and locating services within the community.
Eligibility: Individuals ages 60 years and older residing on the Westside of Syracuse
Application Process: Appointment required; call for information
