Eligibility: Children in 6th through 8th grades residing in the Syracuse City School District

Required Documentation: Proof of residency, proof of age, immunization records, physical examination records, custody papers (if applicable), latest report card or transcript (for entering grades 1-12) copy of most recent IEP (if applicable), evidence of custody or guardianship (if applicable)
