Assertive Community Treatment (ACT) Team

(Program)

Provides services to individuals diagnosed with severe mental illness and whose needs have not been well met by traditional service delivery approaches. Provides an integrated set of treatment, rehabilitation, case management, and support services delivered in the person's home/community by a mobile mental health treatment team.

Eligibility: Individuals ages 18 years and older with severe mental illness
