Transitional Living Crisis Bed Program

(Program)

Provides short-term crisis support in order to avert an unnecessary psychiatric hospitalization, prevent a lengthy hospitalization, and/or provide step down support following hospitalization prior to return to independent living status or while awaiting alternative community placement.

Eligibility: Individuals ages 18 years and older with a severe and persistent Axis 1 diagnosis needing a short-term stay for 7-14 days and have a selected living situation available to return to after stay in program

Application Process: Referral from a NYS licensed psychiatrist required

Required Documentation: Current psychiatric assessment, most recent inpatient/outpatient records, current physical exam, current insurance information
