Health Home Care Coordination
Provides care coordination services for adults, children and families. Supports include scheduling doctor appointments, help with arranging transportation, medication management, access to specialists & providers, support system engagement, and connecting with educational supports.
Program Information
Eligibility: Individuals must:
1) Currently have active Medicaid
2) Have 2 or more chronic health conditions, OR be living with HIV/AIDS, OR have a serious mental illness
Application Process: Call for information and services
Required Documentation: Medicaid ID card and documentation of diagnoses
Fees: None
Service Area: Cayuga, Madison, Monroe, Onondaga, Oswego, and Wayne counties
Languages: Interpretation services available
