A care coordinator can be available to assist individuals in enrolling in programs and services. Both short and long term follow along is available.
Program Information
Eligibility: A person must have a documented diagnosis of a developmental disability.
A person's needs for assistance must be short or long term.
Medicaid enrollment is not required.
