Program Information
Eligibility: Individuals who have had a heart attack, an angioplasty with or without a stent, cardiac bypass surgery, valve surgery, a heart transplant, or angina
Application Process: Physician referral required
Required Documentation: Call for required documentation
Fees: Call to verify insurance information
Service Area: Broome, Cayuga, Cortland, Jefferson, Lewis, Madison, Oneida, Onondaga, Oswego, and St. Lawrence counties
Languages: Interpreter services are available 24/7 for those who are hearing impaired or speak limited or no English
