Program Information
Eligibility: Individuals and families with child(ren) from birth to 17 years of age
Application Process: Must register in-person
Required Documentation: Benefit Summary Sheet and picture ID; if no Benefit Summary Sheet: Social Security cards for all family members, proof of eligible income, proof of ages for all children, proof of address, and picture ID
