Provides services for individuals with developmental disabilities including service coordination and supervised living services through Independent Residential Alternatives (IRA).
Program Information
Eligibility: Individuals ages 18 years and older diagnosed with a developmental disability prior to age 22 who are Medicaid-eligible
Application Process: Call or go online to apply
Required Documentation: Most recent physical exam and psychological report, individua's current ISP, completed consent for release of information form and SBE precautions (if applicable)
