Program Information
Eligibility: Individuals in Cayuga, Madison, Onondaga, and Oswego counties with a life expectancy of 6 months or less
Application Process: Call for information and services
Required Documentation: Documentation of illness and terminal prognosis
Fees: Covered by most insurances, no one refused for inability to pay
Service Area: Cayuga, Madison, Onondaga, and Oswego counties
Languages: Interpretation services available as needed
