Housing First

Works in conjunction with Assertive Community Treatment (ACT) team (through Onondaga Case Management) to provide housing for individuals with chronic/severe mental illness. ACT provides all services/treatment and Helio Health provides a rent stipend.

*Please do not provide this resource to individuals who are seeking housing resources. Must be referred to the program by SPOA.. Program does not accept self-referrals.

Eligibility: Individuals with chronic/severe mental illness who are already approved for SPOA. Individuals are not able to self refer.

Application Process: Appointment required; call for information

Required Documentation: Proof of mental health diagnosis, Single Point of Access (SPOA) referral form
