Provides mom and baby care, surgical care, elder care, and services for individuals who have experienced a stroke, or who have diabetes, heart disease, or breathing disorders. Offers a 24 hour on-call nurse to address patient issues, take referrals, and provide guidance.
Program Information
Eligibility: In-home services provided for individuals who are:
- elderly
- have suffered a stroke
- need post-surgical care
- have breathing disorders, heart disease, or diabetes
- pregnant/postpartum women and their babies
