Jefferson County Home Management Unit
Responsible for authorizing personal care services to Medicaid-eligible individuals who require in-home assistance with housekeeping and/or personal care. Services provided include housekeeping chores, errands, laundry, meal preparation, assistance with personal hygiene, dressing, and grooming. Unit is also responsible for administering the Long Term Home Health Care Program, designed to meet the needs of the chronically ill and disabled person in their home.
Program Information
Eligibility: Medicaid-eligible individuals who require in-home assistance in order to stay in their home
Application Process: Services are authorized after a physician indicates the serves are medically necessary, and a nursing and social assessment indicates the services are appropriate to meet the client's needs
