Maternal / Child Home Visits: Division of Healthy Families
Provides outreach and home visitation to high-risk women, men, and their families. Offers assistance and advocacy for clients to overcome barriers to accessing needed services including help finding a doctor or other medical provider, help with enrollment in a health insurance plan, information on reproductive health, and referrals for help with issues such as relationship violence, substance abuse, and mental health.
