Medicaid Application Assistance Program
Provides one-on-one assistance to help individuals enroll in government sponsored health insurance programs, such as Medicaid, Homecare Medicaid, Nursing home Medicaid, and Medicare Saving Programs. Offers home visits as needed.
Program Information
Eligibility: Individuals who are either age 65 years or older, blind or visually impaired, or disabled
Application Process: Appointment required; call for information
Required Documentation: Proof of identity, proof of address, proof of income, copies of insurance cards, copies of medical bills, proof of resources
Fees: None
Service Area: Franklin, Jefferson, Lewis, and St. Lawrence counties
Languages: Interpreter services available upon request, American Sign Language
