Onondaga County Single Point of Access (SPOA)

Processes applications to determine eligibility for  intensive community based Health Home Care Management, Assertive Community Treatment (ACT), Peer Support and/or Housing services for adults with severe mental illness.

Eligibility: Individuals 18 years and older with a serious and persistent mental illness resulting in functional impairments/disability which seriously interfere with the ability to function independently for an extended period (at least 1 year)
