Provides social day services to support the cognitive and physical abilities of seniors, and helping them remain in their homes. Offers nutritional support, socialization, personal care needs, therapeutic recreation, case management services, medication reminders, diet modification, homemade meals/nutritional support, RN case coordination and assistance with transfers.
