Provides a broad range of programs and services to support people with any disability of any age to maximize independence and community integration. Services include Americans with Disabilities Act consultations and architectural assessment, Chronic Disease Self-Management Program, Supported Employment, Benefits Planning and Advice, Information and Referral, Peer Support both one-on-one and in groups, Individual Advocacy and Systems Advocacy, Community-Based Activities, Educational Advocacy, Family Engagement Events, Community Health Workers, NY Connects and Long Term Care Ombudsman Program.
Program Information
Eligibility: Individuals in Jefferson, Lewis, and St. Lawrence counties with a disability, their friends, family, and community businesses.
